Interior design focuses on the basic structure of a space - really good interior design will re-imagine that space to make it reflect the lives and tastes of those who inhabit it. In fact, if a good design doesn't change your life it should at least change how you live. In its simplest form, Interior design lays a plan for an entirely new space. This is why you'll find designers who offer a service called "space planning". Frankly, I feel that space planning is a critical part of the design process, not an additional service.
